Access medical records
MultiCare Mary Bridge Children’s provides parents or other authorized persons with access to patient medical records. These records may include medical images, test results, allergy information, medications taken, treatment plans and past visit details, among other types of information.
You may obtain medical records in one of two ways: through MyChart or by requesting copies.
Option 1: How to use MyChart to access your child’s medical records
To view your child’s medical record, you will need proxy access through MyChart, which is Mary Bridge Children’s and MultiCare Health System’s secure, web-based patient information portal.
You will need your own MultiCare MyChart account to gain proxy access. Teens 13-17 require their own MyChart account.
To get started, sign the electronic consent form at your child’s next visit to Mary Bridge Children’s.
- If you have a MyChart account: Your account will be connected to your child’s medical record once you’ve signed the consent form.
- If you do not have a MyChart account: A staff member can help you sign up in the clinic. Or, request an access code and complete your sign-up at multicare.org/mychart.
- If your teen (13-17) does not have a MyChart account: A staff member can help them sign up in the clinic.
Once you’ve created a MyChart account and signed the form, your account will be linked to your child’s medical record or your teen’s MyChart account.
Please note, children 12 years or younger cannot have a personal MyChart account. Their medical information will be linked to the proxy’s account.

Option 2: How to request medical record copies
There are two ways to request medical record copies: through MultiCare’s secure online portal or by completing a release form. Please note that you may only request copies of your own medical records through the portal. If you are requesting medical records for someone else, including your child, then you will need to follow the steps below for completing a form (or sending a letter).
Washington state law allows up to 15 days to respond to a medical records request, but in most cases we will process your request within five business days. Depending on the nature of your request, processing fees may apply. Learn more by reviewing our patient medical record copy fee document.

Visit our secure online portal to request copies of your own medical records.
Complete a release form or send a letter
To request a copy of your child’s medical records, complete a release form or send a signed, dated letter specifying what information is to be released and to whom. The release form is also available in Spanish, Russian and Vietnamese with instructions included.
Please note that a patient’s signature is required to release some health information (see the “Consent for Minor” section on the release form for more details).
After you have completed the release form, please return it to one of the departments below. Where you send the completed form depends on the type of records you are requesting. Contact information for each department is listed below.

Requests for imaging records
Imaging records refer to X-rays, MRI scans, CT scans or similar items. There is no cost associated with requesting these records.
To request imaging records, return the completed form to the Central Imaging Records Management Department:

Verbal communications form. This form authorizes health care personnel at Mary Bridge Children’s and other MultiCare facilities to discuss health and payment information with others.
